At Operation Hope DSM, is located in Des Moines, Iowa we are dedicated to supporting our community by providing clean safe short term housing for homeless individuals who are in the hospital but medically stable and need a safe place to recover after hospital discharge.  Our homes are designed to bridge the gap between hospital care and independent living by offering structured, short-term recovery housing in a safe and supportive environment. We partner with hospitals, case mangers, and community organizations to help individuals recover while preparing for their next step towards stability and independence. Each home includes furnished bedrooms, a shared kitchen, and access to laundry facilities and Telephone access. 

1. Referral submitted by hospital or partner
2. Same-day eligibility screening
3. Approval and coordination
4. Moving in within 24 hours

Referrals are reviewed the same day. Placement is often available within 24 hours.

Not Appropriate For This Program

This program is not designed for individuals who:

• Require hospital-level care

• Require 24-hour supervision or nursing services

• Require behavioral health placement or are a risk to self and others

• Require ongoing medical monitoring

Residents agree to:

• Follow all house rules
• Participate in a structured living environment 
• Respectful other residents and staff 
• Participate in case manager plans

Operation Hope DSM maintains structured housing environments with house rules and all program admission will be screened  for background and registry before approval to support stability and safety for all residents.

We provide a reliable housing solution for medically stable patients who cannot be safely discharged.
