Operation Hope DSM provides short-term housing that supports fast, stable transitions while patients await placement.
Safe and stable transition for Individuals who are:
✔ Medically cleared
✔ No safe place to recover
✔ Independent (ADLs)
✔ Able to self-manage medications
We provide clean, structured housing environments designed for individuals who need temporary stability after hospital discharge or during life transitions.

Operation Hope DSM provides housing only and does not offer medical care, supervision, or clinical services.
Residents must be independent and able to manage their own daily needs, including medications.

Medically Cleared for Discharge
The individual has been medically cleared for discharge by a licensed provider and is appropriate for a non-medical, independent living environment.
Operation Hope DSM does not provide medical care or monitoring and relies on the referring provider to confirm medical clearance.

Independent with Activities of Daily Living (ADLs)
The individual is able to independently perform activities of daily living (ADLs), including bathing, dressing, toileting, feeding, and maintaining personal hygiene, without assistance.

Able to Self-Manage Medications
The individual is able to independently manage and take prescribed medications as directed. Medications must be in the individual’s possession or available for pickup at discharge.
Operation Hope DSM does not administer, store, or manage medications.

Ambulatory (Mobility Requirements)
The individual is able to ambulate independently, with or without assistive devices (e.g., walker or cane), and can transfer in and out of bed without assistance.
The individual must be able to safely navigate/live in a shared living environment.
Operation Hope DSM is a non-medical housing option. The following individuals are not appropriate for placement:
To support a safe and appropriate shared housing setting, Operation Hope DSM conducts limited screening using publicly available records, including background and registry checks, as permitted by applicable laws.
Placement decisions are based on safety considerations relevant to a shared housing setting and overall program fit.
Operation Hope DSM does not conduct investigative services and does not provide supervision, monitoring, or clinical assessment.
All screening information is used solely to determine appropriateness for shared housing.
